Accident reports are no longer offered online.
The Plymouth Police Department will no longer allow the inspection or release of accident records without the review and redaction of personal information as described in the Federal Drivers Privacy Protection Act, Title 18, USC 2721-2725 unless the requester is the insurer.
Drivers may obtain a crash report by:
- E-mailing the State of Wisconsin Department of Transportation at: firstname.lastname@example.org Or, you may contact them Monday thru Friday, 7:30 A.M. to 4:30 P.M. at 608-266-8753. You will need the following information: The “DOT Document Number” printed on the upper right corner of the Wisconsin Motor Vehicle Driver Exchange of Crash Information sheet given to you by the officer, or the date of the accident and the name and driver’s license number of one of the drivers. Requests may take up to 30 days to be received from the State.
- Requesting a copy of the report from your insurance agency.
- Requesting a copy in person from the Plymouth Police Department (All personal information will be redacted) Photo ID required at time of request.
